Primary & Preventive Care
Chronic Disease Management
We provide ongoing care for chronic conditions such as heart failure, COPD, diabetes, and dementia.
Annual Wellness Visits & Preventive Care
We provide Annual Wellness Visits, preventive care and recommended immunizations right at home.
Acute and Urgent Care
From a fall to sudden confusion or a fever that won’t break, we’re available for urgent, non-emergency needs, often without a trip to the emergency room.
Clinical Services at Home
Lab Work & Mobile Imaging
We coordinate in-home lab work and mobile X-rays and imaging through our partners.
Specialized Wound Care
Our team includes a certified wound care nurse practitioner who provides specialized care for wounds that need ongoing attention.
Medication Management
Your provider reviews your medications at every visit and can bring in our clinical pharmacist when a regimen needs a closer look. If you need a prescription, we send it to your preferred pharmacy.
Care Management & Coordination
Chronic Care Management
Our care team checks in between visits by phone, monitoring how you’re doing, adjusting care as needed and making sure small issues don’t become big ones.
Care Coordination
We work with your specialists, hospitals, home health providers and Visiting Nurse Agencies (VNAs) to help keep everyone involved in your care connected.
Transitional Care Management
After a hospital stay, we typically visit within seven days of discharge to review medications, assess how you’re doing, and help coordinate follow-up care.
Supportive Services
Behavioral Health & Social Work
We routinely screen for behavioral health concerns and can connect you with a behavioral health provider when additional support would help. Our social worker can also help with care planning, emotional and practical needs, and connections to community resources.
Advance Care Planning
We help you and your family talk through your healthcare wishes and goals so your care reflects what matters most to you.
Palliative Care
For patients with serious or life-limiting illness, we provide care focused on comfort, symptom management, and quality of life.
Hospice Coordination & Referral
When hospice may be appropriate, we help patients and families understand their options and coordinate the next steps.
