Wherever the referral comes from, the care starts the same way.
Primary Care Physicians
You’re managing medications and decisions by phone, without being able to examine your patient. We take over their primary care and see them at home. Your patient may continue to see you if they wish.
Specialists
Your patient has become homebound and is relying on you for more than your specialty was meant to cover. We take on their primary care and coordinate with you on an ongoing basis.
Hospital Discharge Planners & SNFs
Discharge is the start of the transition home. We see patients promptly after discharge, catch problems early, and help prevent the readmission you’re working to avoid.
Home Health Agencies & VNAs
Care can’t move forward without a physician who signs the plan of care and stays engaged. We review and sign orders promptly, and work directly with your team.
