ABOUT US

We’ve been doing this a long time.

Home Reach began with physicians who kept running into the same problem: patients who could no longer get to the office, and who had quietly stopped getting care. Fifteen years later we are still going to the door.

Why we come to you

An exam room tells you part of the story. A home tells you the rest.

We see the pill bottles on the counter, the stairs that have become a problem, what is actually in the refrigerator, and who is really doing the helping. We meet the daughter who drives in on weekends and the aide who is there every morning. Fifteen minutes in someone’s kitchen can explain a year of missed appointments.

That is not a gentler kind of medicine. It is a fuller picture — and it is why care at home works.

How Home Reach got here

Home Reach is older than its name.

Home Reach actually started in 2010 when the physicians at Vanguard Medical Group realized that they were not able to adequately care for their aging patients who had become homebound. The home visit part of the practice initially served Vanguard patients and the parents of Vanguard patients. Over time, Vanguard expanded the service to care for homebound patients in their community.

Today Home Reach is the evolution of that service and has become its own practice now called Home Reach, serving patients in multiple counties in Northern New Jersey.

What has never changed is the model: a physician-led team that comes to the patient.

Large enough to matter.
Small enough to know your name.

Every Home Reach patient is cared for by a physician-led team of nurse practitioners and physicians. Our providers live and work in the towns they drive to, and they carry a panel small enough to actually know the people on it — so when something changes, someone notices.

We are large enough to cover multiple counties in Northern New Jersey, answer the phone and stand behind our numbers. We are small enough that your provider knows who you are before they knock.

The part that isn’t medicine

Early on, our team kept finding the same thing behind the front door: patients who had gone weeks without a hot meal, homes that were no longer safe to live in, and no one checking in. One of our nurse practitioners stood in a patient’s kitchen and made her scrambled eggs, because that was what the visit needed that day.

It is why the work has never stopped at medicine. Primary care at home means seeing the whole situation — and saying something when the problem is not a medical one at all.

That same commitment runs through Connections at Home, a nonprofit founded by Dr. McCarrick. It gives homebound seniors a free computer or tablet, a social worker who checks in regularly, and a way to reach the community resources they need. To learn more or to help, visit www.cahnj.org.
