Our care model is designed for the members who drive the most cost and risk in a value-based arrangement: multiple chronic conditions, frequent hospitalizations, complex medication regimens and no reliable way to get to care.
- Physicians and advanced practice clinicians, including a certified wound care nurse practitioner

- A holistic palliative care approach to patients with advanced chronic illness
Our Medical Director also serves as medical director for a local hospice program, which shapes how we manage members with serious, life-limiting illness.
Infrastructure built to operationalize a contract
We use Lightbeam Health Solutions to receive payer claims and ADT feeds and to power our population health analytics, giving us real-time visibility into hospitalizations and ED visits across our panel, and the infrastructure to manage a value-based contract, not just accept one.
